Service Description

In-home holistic assessment (up to 2 hrs) that will cover safety, physical routines, functional mobility, cognitive capacity/function and application, environmental setting concerns and family support available. We will discuss what is the ideal situation for your loved one and what the family and individuals goals are. What does "thriving" look like for you and your loved one.
